Acute Rheumatic Fever

JONES Criteria for Diagnosis

J - Joint pain migrating to each joints O - Endocarditis N - Nodules of Skin E - Erythemia with red boarder S - Coria

Aortic Stenosis

Aortic Regurgitaion

  • Aortic blood retuning to LV causes Increse the volume of bload in LV => increase in stroke Volume and systolic pressure without any change in Diastolic pressure => elevated Pulse pressure(systolic - diastolic pressure)
  • elevated Pulse pressure => bounding pulse, pulsating nail bed, head bobbing (hyperdynamic circulation)
  • Later Prolonged Elevated Pulse pressusre causes Eccentric LV hypertophy(only load on LV )

Mitral Prolapse

  • loss of soft elastic property of Mitral valve (MArfan syndrom ,Elhars dhamlos )
  • mid systolic click sound as mitral valve prolapse due to LV presseure, may develop to Regurgitation

Mitral Regurgittion

_ wall weakned by AcuteRheumatic fever

_ blowing sound(blood blowing back to RA)

Mitral Stenosis

  • Chronic Rheumatic fever

COmplicatons

  1. LA blood stasis(LA cannot push all blood to LV ) => pulmonary congestion /Pulmonary hypertension => damage of Blood vessles in lungs => Heart failure cells
  2. RV also need to push against Pulmonary Congestion => R-CHF
  3. RA neeed to push to open => RA hypertrophy => may loss conduction fibres(afrillation) => mural thrombus
