Nerve Block complications¶
Nerve Damage from NErve Block¶
- M/C Chemical neuritis from overdose than required
- pain and varying degrees of nerve dysfunction, including paresthesia or motor or sensory deficit.
- chemical irritation from the anesthetic,
- direct trauma from the needle,
- ischemia as a result of intraneural injection.
Nerve block Dosages¶
Prevent Nerve injuries¶
- proper needle style, positioning, and manipulation.
- keep needle parallel to longitutidal axis of Nerve
- sharp pain / paresthesia needle close to nerve
- 25G best to reduce nerve damage
- 27G has limitied aspiration/break/bend
- +Paresthesia
- on insertion withdraw 1-2mm and inject
- on injecting stop and withdraw
- Nerve surrounderd by sheath and BV
- lidocaine in sheath/BV ⇒ compress nerve /nerve ischmia
- intraneuronal injection Severe pain , Difficulty pressing plunger
- Reposition the needle
- Proper positiong minimally ,painfull, good flow of anesthesia without resistance
Intra-vascualr injection¶
-
Intra-Arterial
- Peripheral vasospam
- ischemia from Epinephrine
- Hematoma
- Direct Pressure for 5-10min
-
Intra-vascular
- Not toxic
- only Blanching of Skin
