Adrenaline Dosages in Children

Age Group Dosage
<6 years 0.15 mg IM
6–12 years 0.3 mg IM
>12 years 0.5 mg IM
  • If hypotensive > rapid infusion of 1–2L (NS).
  • hypotensive Children

    NS in boluses of 20 mL/kg, each over 5–10 minutes, and repeated, as needed. Large volumes of fluid (up to 100 mL/kg) may be required.

Adrenaline Administration for Hypotension

If hypotension persists, follow these steps:

  1. Adrenaline Bolus: 0.1–0.5 mg IV in 1:10,000 dilution
  2. Adrenaline Infusion:
  3. Initial rate: 0.1 µg/kg/min
  4. Example: For a 60 kg man, start at 6 µg/min

Antihistamines

  • H1 Blockers:
  • Chlorpheniramine maleate (Avil): 1–2 mL slow IV/IM
  • Promethazine (Phenergan): 25 mg IV/IM stat
  • H2 Blockers:
  • Ranitidine: 50 mg IV stat, then q8h
  • Glucagon 1–5 mg IV over 5 minutes, followed by infusion of 5–15 µg/min is useful in patients on regular β blockers.
